A V-belt and pulley system was used as the drive system for the fan, but the guard was not in place. The employee put his hand into the drive system to completely stop the fan. His hand got caught between the V-belt and pulley, amputating half of his ring finger and the little fingertip on his right hand. | |||||

An employee hooked an extension ladder onto a main overhead cable line and climbed up the ladder. His task was to replace a secondary cable going from the main cable over the sidewalk to a house. When he cut the old cable, which had tension in it, the cable swung, hit the ladder, and knocked the employee off the ladder to the sidewalk 18 feet below.
